The questionnaire of identification of risk factors in travelers, known as the Cuestionario de identificación de factores de riesgo en viajeros in Spanish, is a new requirement to travel to Mexico during the ongoing coronavirus pandemic.

What is the Cuestionario de Identificación de Factores de Riesgo en Viajeros?

The Cuestionario de identificación de factores de riesgo en viajeros is a travel health certificate for Mexico. It was implemented by the Ministry of Communications and Transport of Mexico through the Federal Civil Aviation Agency and the General Directorate of Transport Medicine, based on the recommendation of the Mexican Ministry of Health.

This online Mexico travel health form has been introduced to reduce the risk of contagion of COVID-19 and health risks among travelers arriving in the country and citizens and residents of Mexico alike. The new measure requires passengers arriving in Mexico to supply contact details and travel and health information through an online questionnaire.

Mexican authorities will then use the information provided to identify high-risk passengers and to contact the traveler in the event that anyone on their flight has tested positive for COVID-19. The ultimate goal is to make flying to Mexico a safe experience for all passengers.

Who Needs a Health Declaration Form to Travel to Mexico?

The travel health certificate for Mexico is now a mandatory requirement for all passengers arriving by air from another country, regardless of their nationality. It must be completed in advance of boarding a flight bound for Mexico. It is possible to fill in the form up to 12 hours before the flight departs.

It is not necessary for those arriving in Mexico via a land border. However, travelers should note that the US-Mexican border remains closed due to coronavirus concerns, and exceptions are only made for a select few travelers.

All air passengers over the age of 18 must register their own individual form themselves. Parents and guardians of minors are able to complete a form on behalf of their child. A single passenger is permitted to carry the QR code of several questionnaires and present them on behalf of their travel companions.

It is necessary to complete a Mexico travel health form for every trip to the country. The QR code generated by the questionnaire is only valid for a single flight.

Requirements for the Mexico Travel Health Questionnaire

The Mexico health declaration form requirements state that applicants must have a valid passport or other accepted ID document. Additionally, they must have already booked their flight to Mexico and have a confirmed boarding pass.

It is also necessary to supply the following information to fill out an online Mexico travel health questionnaire:

The passenger or crew member completing the Mexico travel health form must then provide a signature to certify that all of the information supplied is correct and accurate. All of the data supplied on the form is protected under the Mexican Federal Law on Protection of personal data held by individuals.

Mexico Travel Health Questionnaire Online Registration

After the traveler submits their Cuestionario de identificación de factores de riesgo en viajero, a personal QR Code is generated based on the responses supplied on the form. It is not necessary to print this code, it may simply be shown on a mobile device.

The QR code is issued in one of 2 colors, green or yellow. Travelers who are presented with a green QR code can go directly to airport security controls and board their flight to Mexico. The code is valid for 3 hours after the departure of your flight.

If the QR code generated is yellow, you and any travel companions may be referred to the airport’s medical examination area. You will then need to undergo medical screening before heath professionals determine if you can continue your journey.

Other documentation needed to visit Mexico during COVID-19

A health declaration form for Mexico is currently the only coronavirus-related document that travelers need to visit the country during the ongoing pandemic. Mexico does not currently require arriving passengers to have proof of a negative COVID-19 test, vaccine certificate, or to undergo quarantine to gain entry to the country.

However, all foreign citizens traveling to Mexico for short stays for either tourism or business are also required to pre-register for a FMM tourist card for Mexico. This is a mandatory requirement regardless of whether the passenger is arriving by land or air.
