A number of countries worldwide have introduced a mandatory travel health certificate requirement for passengers in order to pre-screen travelers before they arrive at their destination and contain the spread of COVID-19 within their territory. The Turkish government has also introduced its own health declaration form, known as the Entrance form for Turkey.

What Is the Form for Entry to Turkey?

The Form for entry to Turkey is an electronic health declaration that was implemented by the Turkish government on March 14th, 2021. It is expected to be a mandatory requirement for passengers traveling to Turkey until the end of 2021, at the earliest.

The Turkey travel health form has been introduced to electronically register traveler’s health information and safeguard the health of visitors to the Republic of Turkey and residents alike. It allows the Turkish government to contain the spread of COVID-19 in Turkey by contacting passengers who have tested positive for coronavirus or who have come in contact with an infected individual.

After the traveler completes the application, a copy of the online form is generated, alongside a QR Code and a HES code. HES stands for Hayat Eve Sığar in Turkish, which translates into ‘Life Fits Into Home’ in English.

The HES code allows the holder to share their COVID status with transportation operators. It is mandatory for traveling on public transport between cities in Turkey, including on buses, trains, and domestic flights.

Who Needs a Health Declaration Form to Travel to Turkey?

The Turkey health declaration form is now a mandatory requirement for all passengers arriving from abroad, regardless of whether they are a foreign citizen or a resident or national of Turkey. Only passengers in transit through Turkey to an onward destination and children under the age of 6 are exempt.

The Entrance form for Turkey must be completed before boarding an aircraft or other mode of transportation. It is necessary to present the form and associated codes at both check-in and on arrival at Turkish immigration control.

Unless you hold a passport from one of the few visa-free countries for Turkey, you will also need an approved Turkish visa to gain entry to the Republic. A number of nationalities are now able to apply for a Turkey eVisa online for short tourist or business stays, eliminating the need to visit an embassy or consulate in person to submit a visa application.

The online Turkey visa application only takes a few minutes to complete with basic personal, passport, and travel information. Once approved, the e-Visa is sent to the traveler via email. A copy can then be printed to present to Turkish border officials on arrival.

Turkey Health Declaration Form Requirements

In order to register for a travel health certificate for Turkey, it is necessary to fill out a simple online form with the following information:

  • First name
  • Surname
  • Gender
  • Nationality
  • Passport number and issue/expiry dates
  • Email address
  • Phone number
  • Arrival and departure dates
  • Mode of transport
  • Name of the transportation company
  • Flight number (if arriving by air)
  • Address of accommodation in Turkey
  • Countries visited in the last 10 days

It is essential to double-check that all the information provided is correct before submitting an Entrance form to Turkey. This is because even small errors in the data supplied may result in the traveler being denied entry to the country at a border checkpoint.

How to Complete an Entrance Form for Turkey Online Registration

To complete a health declaration form application, travelers are simply required to fill in the electronic form with the information detailed above.

Once the completed form has been approved, it is necessary to print a copy or download it to a mobile device to present in order to board transportation for Turkey. It must also be presented to immigration authorities upon arrival at Turkish border control, who will scan the associated QR code to confirm its validity.

The HES Code generated at the time of approval of the entry form should be presented at the moment of purchasing any tickets for domestic intercity travel within Turkey. The HES code information will be used to contact the traveler in the case they have come into contact with anyone positive for COVID-19 during their stay in Turkey.

Other Entry Requirements for Turkey during COVID-19

In addition to the need to complete an online Form for entry to Turkey, travelers also need to meet some additional document requirements to visit the country during the ongoing coronavirus pandemic.

At present, passengers arriving in Turkey must also present proof of a negative COVID-19 test. Travelers arriving from most countries are able to either have a negative COVID-19 PCR test taken at most 72 hours before arrival or a negative COVID-19 rapid antigen test taken at most 48 hours before arrival, with the exception of children under the age of 6 and merchant seaman.

These passengers are exempt if they present a COVID-19 vaccination certificate showing that they were fully vaccinated at least 14 days before arrival, or a COVID-19 recovery certificate issued at most 6 months before arrival. Passengers arriving from Hungary or Serbia with any COVID-19 vaccination certificate issued by Hungary or Serbia, and any accompanying minors younger than 18 years are also exempt.

However, passengers aged 6 and older arriving from the following countries must have a negative PCR test taken at most 72 hours before arrival:
